Complaints Form

We make every effort to give the best service possible to everyone who attends our practice. However, we are aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint.

Complaint Easy Read Guide

Please note ! Where a complaint is made by a third party on behalf of a patient, written consent from the patient is required to allow us to investigate and respond.

Please return the signed third-party consent form via Engage Consult, by post, or in person at the practice.

Please note: our website does not currently allow documents to be attached to the online complaints form. If you wish to submit both the complaint and the consent form online, please use Engage Consult, as this platform allows documents to be uploaded.

30.1.26 - Please note that the online complaints form is currently unavailable. We apologise for any inconvenience caused.

Leave feedback via NHS.UK

Your review must be about a specific experience and not a general view of the NHS or the service as a whole.
